Medical Social Worker
Position type available is Full-time or Part-time
Compliant with all applicable federal, state, local and Medicare laws and regulations.
Provide referral to community resources to assist patient and family.
Participate in the development, implementation, review and revision of the plan of treatment as a member of the home health team.
Provide consultation on social/psychological issues to other team members.
Be a resource and support person for the patient, family, aides and BIHC in regards to care and services provided.
Participate in a case conferences, meetings, In Services and training programs as required to ensure professional growth.
Position Requirements
Must be a licensed MSW
Minimum of 1-year social work experience
Minimum of 1 year home health experience
Position Summary
Licensed Medical Social Worker (MSW) ensures Bowes In Home Care (BIHC) is compliant with all applicable federal, state, local and Medicare laws and regulations in providing significant information about the resources and support available in the community for the patient. Provide psychotherapy, counseling and other psychological approaches to help a patient to expand and strengthen their network of social support in accordance with the skill set of a Medical Social Worker. Ensures all service provided is completed according to BIHC standards and expectations.
